The following forms are available to our riders for your convenience:
New Rider Application Form – to be completed by each new rider applicant.
Session Registration Form – complete for each session.
Rider Information Update – complete yearly prior to the Fall Session (September).
Rider Consent Forms – complete upon request.
Physician Assessment Form – complete before your initial assessment and then yearly prior to the Fall session (September). The Physician Assessment must be completed by a Physician and faxed or delivered to ManeGait.
Financial Assistance Form – complete to apply for financial assistance and yearly prior to the Winter session (January).
Information Release Form – complete to request rider progress notes or other information to be released to an outside source.
